If your form is not working, check these solutions.
Check the Group Form object
Click on the form and move it. The entire form should move together. This includes the form field, the background, the graphics, and anything else you want on the form.

1. Label and field colors

The fields become a form when they are grouped together. Grouping creates a form object with stored information about the form and the fields within the form.

Tab order creates a logical order that takes visitors through the form when they hit the tab key. Only the fields you select are included in the tab order. You do not need to include fields that do not request text, such as radio buttons or checkboxes.
